What you are asking is a complete Project Setup/Implementation along with User Management with defined roles & enablement of Survey functionality which involves several things to be done on C4C system. .
This certainly cannot be done over the posts here or through emails.
I would suggest to check documentation on implementing C4C along with specific functionality on Surveys.
